Allergy Medication Form - This form will allow your student to have an epi to be either kept in the office or carried with them.
Asthma Medication Form - This form will allow your student to have an inhaler either kept in the office or carried with them.
School Medication Form - Medication to be administered from the Office.
Education Vacation Leave Form - Use this form to request an absence due to vacation and to request schoolwork in advance. Please submit the form to the office at least 5 days before the vacation to allow teachers time to prepare materials.
